Community Health Partnerships
Community Health Partnerships (CHP)
A wholly owned subsidiary of the Department of Health that is responsible for the delivery of LIFT and takes a stake in each LiftCo (the public private partnership vehicle used in a LIFT).
Previously known as Partnerships for Health or PfH.
